Send a message via the Form

The Form is a Google service. It is important to note that the structure of the form (e.g., field layout, block at the top) is provided by Google in a predefined format. This part cannot be modified, and we cannot transform it.

Using a Google account

Using the Form does not require a Google account! If you are not logged in, ignore the second line of the top block. If you are logged in, Google will display the email address associated with the account in the second line.

If you have a Google account, are logged in, and would like to use Google's special services (e.g. viewing your own answers, editing options in certain cases, etc.), learn about them using the detailed information on Google's official pages (click on the blue link in the top block of the form).

The role of the asterisk (*)

The third line of the top block of the form indicates that the fields marked with a red asterisk on the form are required. They must be filled in, otherwise the form will not allow submission.

Required fields

We have marked the name and email address fields as mandatory so that you don't forget to fill them in! Because we can only respond to the message you send if we know them.
If someone wants to remain completely anonymous, they can enter a "pseudonym" and a non-real email address - the system will accept this, but will not check it. (Please indicate in the email that we should not try to send a reply!)

Data management

Google Forms does not send us any background information (e.g. IP address, account details) about the form filler. We only receive what you enter into the fields.
